​If you would like to become a parishioner at Good Shepherd please obtain and fill out the green registration form found on this page. You may also pick a physical copy up at the Church. They are located in the vestibule of the church.  Please return the completed form to the parish office or give to an usher when you come to Mass.

You may also notify the parish office if you wish to register with, or be removed from the parish records, or if you have a change of address, telephone number, or a new addition to the family.
